In Massachusetts, a person who has received appropriate medical intervention for gender transition, and has had a legal name change by a court, may have their birth record amended to reflect the newly acquired sex and name.
The applicant must submit to the appropriate clerk a certified copy of the legal name change court order and can only do a name change when changing the sex marker.
The applicant must provide the town clerk (in town/city of birth) with a physician’s notarized statement indicating completion appropriate medical intervention for gender transition.
